Xenia Community Schools is committed to the health and well-being of every student, and our goal is to provide a safe and healthy environment for our school community. The district has three full-time nurses and eight clinic aides to ensure that students receive minor treatment for any simple concerns, as well as provide maintenance care for any chronic conditions. Our staff members manage health screenings and oversee your child's medical records.
If you have questions about any of these forms or clinic procedures listed on this page, please reach out to your school clinic staff member through the main office.
When to stay home...
As part of our goal to create a safe environment, we ask parents and families to help us maximize the time that students are healthy and ready to learn together! Children should stay home if they are experiencing a fever of 99.4 (if taken by forehead; 100.4 if taken by mouth), vomiting (within the past 24 hours), diarrhea (within the past 24 hours), experience a body rash with itching or fever, eye infection, head lice, or have been to the hospital.
When to return to school...
Students may generally return when they have been fever-free without the use of fever-reducing medication for 24 hours, and no vomiting or diarrhea (for 24 hours). Students with other issues such as rash, eye infection, or a hospital/ER visit should be evaluated and cleared to return by a doctor. Students with head lice must be treated and approved to return by clinic staff.

Don't Forget!
If your child is ill, you MUST report the absence to their building first thing in the morning.
Families must also follow the call with a written note.
If your child accumulates ten (10) absence days or more in the current school year, a note from your child’s doctor is required. It should be submitted within five (5) days of your child’s return to school.
A Word About COVID-19
The district continues to follow the direction of Greene County Public Health and the CDC. Masks remain optional for those who do not exhibit symptoms of COVID or have been in direct contact with a confirmed case. As always, families should notify their school if their student or a family member (in the household) exhibits symptoms of COVID.
With certain exceptions per XCS Board policy 5320, every student must meet Ohio's Immunizations for School Attendance Guidelines.
If you wish to apply for an exception, families should submit a State of Ohio Legal Immunization Exemption form annually for each student.
While most forms for the district are collected through Final Forms, there are a few clinic authorization forms that must be completed and submitted each year, if needed:
Dental Care Forms
Once again, Xenia Community Schools will offer both dental and vision screenings for our students in partnership with Health Partners of Western Ohio. Following each screening, follow-up will include any needed services. For vision, this could include glasses (if needed), while for dental screenings, this could include cleaning and protective sealant.
Remember that a parent/guardian must complete the consent form for your child to be treated. The provider cannot see your child without this consent.
These services are at NO OUT-OF-POCKET COST to families, whether or not you have insurance. Insurance will be billed, if available.
NEW for 2024, Vision Screenings will take place in the fall! We are excited that vision screenings will take place earlier this year. However, this means that a parent/guardian must complete and return the consent form to receive vision care. Printed forms are also available in your child's school.
Questions? Please contact your child's school clinic directly!
In the fall of 2022, Xenia Community Schools began a pilot program in partnership with Dayton Children's to offer a school-based health center (SBHC), housed at Warner Middle School.
Our school clinics will work hand-in hand with the school-based health center to improve health — and educational! — outcomes for Xenia students by keeping them in the classroom and learning together.
As they have always done, our school nurses and clinic aides will continue to support day-to-day student health in a variety of ways, while new services and supports for Xenia families will be provided by a nurse practitioner from Dayton Children's.
Visit the SBHC information page to learn more about this exciting new program.
