All families with a student who receives special education services have been asked to participate in a short, 25 question survey through the Ohio Department of Education (ODE) in order to improve services for students with disabilities across Ohio. 

Families may be eligible to participate in the survey if their child has an IEP and receives special education services. If you are eligible to complete the survey (which you can find out by beginning the survey itself), and you enroll in the study, you will be entered in a drawing for one of several $50 Amazon gift cards. Your chance of winning a gift card is approximately 2.0%.
