Final Forms Directions

Final Forms Directions for 2019-20

The steps noted below must be followed to complete the beginning of the year Final Forms document collection process:

1.  The student's Parent/Guardian must sign onto Final Forms to review and electronically sign the necessary documents; AND

2.  After the Parent/Guardian has electronically signed the documents, the student must then sign onto Final Forms to review and electronically sign the documents that the Parent/Guardian has signed.

Only after both the Parent/Guardian AND student have electronically signed the documents online, is the Final Forms beginning of the year document collection process complete.

Parents/Guardians please note that you may log onto Final Forms at any time throughout the school year to update information regarding your student.  Please note, however, that for any address changes and custody changes, documentation must first be submitted to the Board Office.
